SNE-PTN Clinical Quality Measures

Clinical Quality Measures, or CQMs, are tools that help to demonstrate quality in healthcare delivery. Collecting, reporting and using data for quality improvement is a key activity of SNE-PTN.

Practices will collect and report data on at least one clinical quality measure on a quarterly basis.

SNE-PTN Clinical Quality Measures are separate from MIPS measures.

Practices can report on the Asthma clinical quality measure.

Practices can report on the CAHPS Patient Survey Adult clinical quality measure.

Practices can report on the Diabetes clinical quality measure.

Practices can report on the High-Risk Medications clinical quality measure.

Practices can report on the Imaging Studies for Low Back Pain clinical quality measure.
