Our network works with primary care and specialty practices to customize practice transformation plans that help improve the quality of care provided to patients and enhance use of electronic health records. We work with practices every step of the way, coaching them on new care delivery models, sharing best practices and assisting in implementing alternative payment models tied to quality or value. We’re committed to our practices long-term success.
Massachusetts’ and Connecticut’s public medical schools are leading this effort through their regional health care systems. Participating practices benefit from shared learning and free access to faculty and community experts and resources. One of our goals is to help practices strengthen connections to community partners.
