The Southern New England Practice Transformation Network (SNE-PTN) is one of 29 practice transformation networks across the country chosen by the Centers for Medicare and Medicaid Services (CMS) to be part of its Transforming Clinical Practices Initiative (TCPI). The collaborative is led by the University of Massachusetts Medical School and UConn Health.

The network supports primary care and specialist clinicians – from optometrists to dentists to social workers – to improve the health of their patients and financial health of their practices through technical assistance and shared learning opportunities. Participating clinicians will be better positioned for payment reform.

The future success of medical practices depends on health care quality and efficiency. This collaborative will serve as a champion for continued improvement, culture change, and patient and family engagement.
