Why Participate

Bring joy back to your clinical practice. Clinicians who join the Southern New England Practice Transformation Network (SNE-PTN) will improve the experience of care for their patients and themselves, leading to enhanced health care quality and efficiency. The financial health of participating practices also will benefit as the clinicians learn to adapt to alternative payment models tied to quality or value. There is no financial cost to sign onto the network, one of the best reasons to enroll.

The network will support clinicians by providing technical assistance from quality improvement advisors, coaching and mentoring through shared learning opportunities, and tools to optimize the use of electronic medical records. Participants will receive customized help developing core competencies specific to their practice transformation goals.

The network aims to make care affordable, promote effective prevention and treatment of chronic disease and reduce unnecessary hospitalizations, testing and procedures.

Eligible clinicians are professionals of all specialties who bill Medicare, Medicaid, or CHIP. Clinicians who receive federal funding through a Medicare ACO are excluded from participation.
